NHS Health Checks GP and Pharmacy Liaison Lead

We are looking for an enthusiastic, motivated NHSHC GP and Pharmacy Liaison Lead, to work within our integrated Healthy Lifestyle Service, as an integral part of our Staffordshire service. The ideal candidate will be required to work in the Staffordshire area.  You would need to have a driving licence, access to a car and be open to travel and support teams throughout the county as required.

This role will be working with GP’s and Pharmacies across Staffordshire to implement and support subcontracting of the delivery of NHS Health Checks within their organisations and to co-work alongside our Stop Smoking Pharmacy Lead to continue to expand the stop smoking offer within pharmacies. If you are an excellent communicator, who is enthusiastic, motivated, and looking to make a difference to local communities, then this is the role for you.

The key functions of the role are:

To increase the number of NHSHC referrals by working with GP’s and Pharmacies across Staffordshire, ensuring quality data returns and to ensure referral pathways for all the integrated services are being implemented.

The post holder will:

  • Arrange meetings with GP’s and pharmacies to discuss opportunities that we can offer and onboard these as our subcontractors to deliver the NHSHCs 
  • Ensure all relevant policies and procedures are in place 
  • Ensure that training, including STPs and Making Every Contact Count (MECC), is completed 
  • Assist GPs with searches for eligible patients and uploading of data.
  • Audit the subcontractors on delivery and quality of data submissions 
  • Manage SLA’s and the monthly data submissions and finances with subcontractors  
  • Manage relations with the GP’s and pharmacies to ensure consistency and data quality is being adhered to with monthly visits/calls  
  • Report to Deputy Service Manager weekly with general service overview and highlight any issues as and when needed 
  • Weekly update meetings with Stop Smoking Pharmacy Lead 
  • Attend relevant events such as PCN events.  

The successful candidate will:

  • Have strong interpersonal and communication skills
  • At least a minimum of 2 years’ experience of working within a health care setting.
  • Good knowledge of System one, EMIS and NHS App.
  • Understand the needs of key target groups in order to develop service provision to meet needs.
  • Provide information on all Staffordshire Integrated Lifestyle services.
  • Establish effective partnerships, working to embed referral pathways and deliver brief advice training to facilitate referral pathways.
  • Contribute to the planning, delivery and continued support of  NHSHCs and Stop Smoking services within the area in a variety of settings.
  • To deliver training to subcontractors to understand the service referral pathway and delivery model.
  • Follow protocols and guidelines for advising service users and stakeholders.
  • Deliver MECC/NHSHC training to all subcontractors.
  • To deliver NHSHCs as part of the NHSHC service.

  • They’ve ever had restrictions placed on their clinical practice as part of the revalidation process
