We are looking for a motivated and compassionate Hypertension Health Coach to support individuals living with hypertension in making sustainable lifestyle changes that improve their hypertension. You will work directly with participants across Bath and Northeast Somerset to facilitate positive behaviour change using evidence-based approaches, tailoring support to individual needs. You will work in collaboration with GP surgeries, community pharmacies, and the Voluntary, Community, Faith and Social Enterprise (VCFSE) sector to establish and maintain clear referral pathways, ensuring eligible can easily access the service.
The post holder will also work with the Integrated Care Board (ICB) to promote shared decision-making between referrers and, supporting the referral of individuals who are motivated and ready to make positive changes to their health and wellbeing.
This role is ideal for someone passionate about prevention, empowerment, and person-centred care.
Key responsibilities:
- Support people with hypertension with structured health coaching.
- To engage with service users referred from VCFSE partners, community pharmacy and/or General Practice of people diagnosed with hypertension who have a BMI>30, smoke or have other behavioural risk factors that contribute to their hypertension.
- Provide holistic support including access to local resources, and referral to other support services such as mental health support and social prescribing, as appropriate.
- Achieve improvements in blood pressure, BMI and smoking cessation.
- Work flexibly to provide a coordinated service across the localities to meet the needs of the client group.
- Use clinical systems, to manage service user data for accurate record keeping.
The successful candidate will be:
- Health Coach Trained and NCSCT trained
- Highly motivated self-starter with resilience, determination, and the ability to see jobs through to completion.
- Managing own workload, to ensure tasks are completed on time and to a high standard
- Evidence of a personal commitment to continuing professional development and to maintaining an up-to-date professional knowledge supporting the development of business within the company.
