Triage Behaviour Change Coach – Bath and Northeast Somerset

Full time role available

We are looking for a motivated and compassionate Triage Behaviour Change Coach to support individuals in making sustainable lifestyle changes that improve their health and wellbeing. You will work directly with participants across Bath and Northeast Somerset to facilitate positive behaviour change using evidence-based approaches, tailoring support to individual needs based at our Everyone Health office, Bath.

This role is ideal for someone passionate about prevention, empowerment, and person-centred care.

Key responsibilities:

  • Support service users with an initial triage into appropriate services provided by the Community Wellbeing Hub , providing empathetic and motivational support, to ensure a professional entry into the service.
  • Provide day to day triage support within Everyone health including handling telephone calls/messages, electronic communication, appointment management and updating service users records
  • Use clinical systems, to manage service user data for accurate record keeping.

The successful candidate will be:

  • Highly motivated self-starter with resilience, determination, and the ability to see jobs through to completion.
  • Managing own workload, to ensure tasks are completed on time and to a high standard
  • Evidence of a personal commitment to continuing professional development and to maintaining an up-to-date professional knowledge supporting the development of business within the company.
